Description
Seeking hospitalized patients at UMass Memorial Heath with Type 2 Diabetes for a study to improve support following hospital discharge.
Overview
Participants in this study will complete an online questionnaire and then will be randomly selected to receive the usual care or be matched with a patient navigator to support their care following hospital discharge. Participants may need to come in for a finger stick HbA1c test if one is not done as part of their usual care.
What we're hoping for
We are studying whether a patient navigator improves recovery compared to the usual care after patients with Type 2 Diabetes are discharged from UMass Memorial Hospital.
