Seeking men and women 65 years of age or older to understand whether a personalized care plan for home exercise, coupled with the use of mobile health technology following a hospital admission for a heart attack or a cardiovascular procedure can prevent decline and improve health.

Overview

We are studying whether a personalized care plan for home exercise, coupled with the use of mobile health technology following a hospital admission for a heart attach or a cardiovascular procedure can prevent a decline in health compared to the usual care alone.

What we're hoping for

To evaluate whether a the personalized care plan for home exercise along with the use of mobile health technology improved functional capacity in older adults compared to usual care alone.
