Study To Identify Risks For Readmission Following Hospital Discharge For Older Adults

Description

Seeking men and women aged 65 or older who have been discharged from the hospital within the past month for a survey of their health and use of healthcare following their discharge from the hospital.

Overview

We are looking at the health records and conducting interviews of adults aged 65 and older to better understand what might increase their risk for readmission to the hospital. Participants will be contacted by a pharmacist for an over the phone interview about what drugs you are taking and how you are feeling and agree to let researchers review their medical record.

What we're hoping for

We hope to identify risks for readmission to the hospital for adults aged 65 and older who have recently been discharged from the hospital.
